What is Feline Distemper / FPV in Cats?

FPV / Feline Panleukopenia Virus, otherwise known as Feline Distemper, is a highly contagious, life-threatening infectious disease in cats. It mostly affects kittens and unvaccinated cats, and it can be fatal if not appropriately diagnosed and treated. It is closely related to canine parvovirus (often confused as it is not related to the canine distemper virus).

The feline panleukopenia virus infects and kills the rapidly growing and dividing cells in the body, including cells in the bone marrow, intestines, and skin, and in a developing fetus.

The virus suppresses the production of all white blood cells in the bone marrow. These cells are important to the immune system and are used to fight infection. Without them, the cat is vulnerable to spreading the virus.

Infected cells in the intestines eventually lead to diarrhea, decreased appetite and vomiting. Severe dehydration ensues and the safety barrier between the intestines and the rest of the body breaks down, leading to secondary bacterial infections. The virus spreads quickly and is fatal, if left untreated.

Feline distemper virus is found everywhere in the environment; it is strong and can live for years. The virus can survive in the environment it was shed into or be carried on shoes, paws, bedding, bowls, litter boxes, etc. It can survive at freezing temperatures and room temperatures, and it can also survive the use of certain disinfectants, including iodine and alcohol.

The virus enters the body through the nose or mouth; the specific cat’s immune system determines the number of viral particles that enter the body. Usually, the virus invades the bone marrow and intestines within two to seven days of the cat coming into contact with the virus.

Panleukopenia is considered one of the deadliest cat diseases in the unvaccinated cat population. Kittens, cats with depressed immune systems and pregnant cats are at the highest risk to develop severe clinical signs of this disease.

Symptoms of Feline Distemper / FPV

Symptoms of feline distemper can range from mild to severe and may include the following:

  • Lethargy

  • Depression

  • Vomiting

  • Diarrhea

  • Painful abdomen (when touched or may appear tense and distended without touching)

  • Dehydration

  • Weight loss

  • Rough or unkempt hair coat

  • Fever

  • Decreased to absent appetite

  • Collapse (in severe cases)

  • Bruising of the skin or gums (in severe cases)

Kittens infected in the womb during the late stages of pregnancy may develop cerebellar hypoplasia once born, a condition where the virus affects the formation of a part of the brain called the cerebellum. This is the part that coordinates balance and movement.

Affected kittens will have mild to severe intention tremors and a wide-based stance. They may fall frequently or seem uncoordinated. Luckily, this is not considered a painful condition, and these special-need kittens, if kept in safe environments, can live long happy lives.

Causes of Feline Distemper / FPV

Cats become infected with this virus in utero due to their mother becoming infected while pregnant or being exposed to it in their environment. Kittens can also be infected in utero or via breast milk when feeding from their mother.

A cat carrying feline distemper virus will shed viral particles into the environment through feces, urine, saliva and vomit. Infection occurs when susceptible cats are in contact with these secretions.

Luckily, an infected cat only sheds the virus for a day or two, but since the virus is very stable in the environment, it can live for years. That is why it is common for cats to become infected due to their environment.

Diagnosing Feline Distemper / FPV

Feline panleukopenia can look like many other conditions, such as feline leukemia or feline immunodeficiency virus. Any kitten showing signs of fever, vomiting, diarrhea, depression, and a decreased appetite is a feline panleukopenia suspect. A thorough medical history can be helpful with diagnosis, especially if your cat was recently adopted, has been outdoors frequently, is unvaccinated, and/or has been around other cats.

Your veterinarian will likely do bloodwork to help with the diagnosis. Feline panleukopenia is suspected when exposure history is correlated with very low white blood cells and possibly low red blood cells (anemia).

Fecal testing for feline panleukopenia virus can be performed . If the cat received a panleukopenia vaccine 5-12 days prior to testing, this form of test is often falsely positive. Virus isolation (a test to find the virus in different tissue samples), antibody levels and PCR testing are also available to help confirm suspicions of feline panleukopenia.

Treatment of Feline Distemper / FPV

There is no specific treatment for feline panleukopenia. Dehydration is treated with aggressive intravenous fluid therapy while clinical signs of vomiting and diarrhea are treated with prescription medications.

Antibiotic therapy is often instituted to help control any secondary bacterial infections due to low white blood cell counts. However, antibiotics are not used to treat and clear the actual virus. For kittens and cats who appear in shock, or with severe infections, aggressive therapy and nursing care is necessary for resuscitation, using plasma/blood transfusions and anti-coagulant therapies.

Luckily, cats who do receive and respond well to aggressive therapy during the first few days of feline panleukopenia tend to have a good prognosis for a complete recovery. Kittens infected in utero during the early to middle stages of pregnancy typically do not survive. Those infected in the later stages of pregnancy tend to develop cerebellar hypoplasia but can live happy lives with normal survival times, depending on the severity of the neurological signs.

Prognosis decreases for cats with low protein levels, low temperatures, thin body conditions, and/or severely low white blood cells on bloodwork. These extremely ill cats have a poor prognosis and, on average, will pass away within 12-24 hours.

If a cat recovers from feline panleukopenia, there is usually no permanent damage to their organs, and they develop lifelong immunity to the virus.

What is Feline Asthma? Clinical Signs, Diagnosis, Treatment, and Prognosis

Asthma is a disease of the lower airways of the lungs that affects between 1 and 5% of cats.

Although there is some debate about the definition, most clinicians and researchers agree that feline asthma is caused by an allergic reaction to inhaled allergens— particles that stimulate a cat’s immune system.

When a susceptible cat first inhales an allergen, their system creates specific antibodies to target that antigen. When the cat is exposed to that antigen again, these antibodies recognize the allergen and start a cascade of events that bring many different types of immune cells to the airways. These immune cells then trigger the production of substances that promote inflammation, a condition that results in irritation, swelling, and reactive constriction of the airways. As a result, the diameter of the airways may decrease and mucus may accumulate within the passages. All these processes limit the ability of air to move through the airways and cause the cat to experience difficulty breathing.

The average age of cats diagnosed with asthma is between 4 and 5 years. It doesn’t appear that one sex is more susceptible to asthma, and although some studies suggest that Siamese cats may be predisposed to asthma, this has not been definitively proven.

Clinical Signs of Feline Asthma

Cats suffering from asthma may show signs of:

  • difficulty breathing

  • wheezing

  • rapid breathing

  • coughing or hacking

  • open-mouthed breathing

  • vomiting

These signs can vary in intensity, ranging from acute respiratory crises to chronic, low-grade coughing, elevated respiratory rate, or increased respiratory effort. These signs may happen spontaneously or they may be elicited by pressing lightly on the cat’s throat area. During an asthma attack, many cats hunch their body close to the ground and extend their necks forward in a characteristic posture

Diagnosis of Feline Asthma

There is no single, specific test that can definitively diagnose feline asthma. Instead, veterinarians rely on information gathering and testing to arrive at a diagnosis. If a veterinarian suspects your cat may have asthma, he or she will take into account the cat’s health history as well as the results of imaging studies, microscopic evaluation of the cells in the cat’s airway secretions (cytology), and in some cases, blood and allergy testing.

When it comes to diagnosing feline asthma, a picture is worth a thousand words, so veterinarians employ radiographs (X-rays), computed tomography (CT), and bronchoscopy to see inside a cat’s lungs and evaluate their condition. In cats with asthma, radiographs often – but not always – reveal a characteristic bright branching pattern along the airways that is created by the accumulation of inflammatory cells. Air trapped in the constricted airways may also cause the lungs to become overinflated and appear larger than normal in a radiograph. CT, which uses X-rays to generate three-dimensional reconstructions of the body, may also be useful in diagnosing asthma and distinguishing this condition from other causes of airway disease in cats, but this application of CT is still somewhat experimental.

Bronchoscopy is a technique in which a flexible camera (a bronchoscope) is passed through the mouth and down into the airways of the lungs. This technique, which requires either heavy sedation or general anesthesia, can be used to visualize the inside of the airways and to collect samples of cells that line the airways. Cats with asthma often have distinctive changes in the appearance of the lining of their airways, although some of these changes may also be seen with other respiratory diseases of cats. Analysis of cells from the airways of cats with asthma may reveal the presence of high numbers of inflammatory cells, although these cells may also be found in cats suffering from other respiratory diseases.

When diagnosing feline asthma, it’s crucial to rule out other conditions that can cause similar signs and give similar test results. Chronic bronchitis, infestation with lungworms, migration of other parasites into the lungs, as well as other infectious diseases that result in pneumonia can all result in signs and test results similar to those found in a case of feline asthma. Drugs used to treat these other conditions may not be appropriate for a cat with asthma, and may do more harm than good.

Treatment of Feline Asthma

To treat feline asthma, veterinarians will usually prescribe corticosteroids to reduce inflammation in the lungs, with or without bronchodilators to dilate the airways. Both of these types of drugs come in oral, inhaled, and injectable forms. Although corticosteroids may be used alone, bronchodilators are usually not used on their own as they don’t treat the airway inflammation that brings on asthma.

Other drugs and treatments for feline asthma have not yet been conclusively proven to work. Experimental therapies, including desensitization to specific allergens (like allergy shots in people), the use of omega-3 fatty acids, and drugs intended to disrupt the metabolic pathways that lead to inflammation show some promise for treating feline asthma, but further research is necessary to refine them and prove their safety and effectiveness. Other approaches are sometimes recommended because they are beneficial in human asthma patients, including purifying indoor air and avoiding allergens, but they have not been rigorously tested or proven to work in feline asthma patients.

Prognosis of Feline Asthma

Feline asthma is often a progressive condition that fails to improve significantly over time, and affected cats may experience occasional asthmatic flare-ups that vary in intensity from mild to life threatening. Although cats can never be truly “cured” of asthma, by carefully monitoring their respiratory effort, keeping an eye out for coughing, and intervening with medication when they need help, owners can help their asthmatic cats live happily for years.

It’s a lot like kids who suck their thumb — once the habit starts, it’s hard to break. Here’s what you need to know about causes and treatment.

A dog’s list of annoying habits can often be endless. Obsessive itching, licking, scratching, urinating on your favorite rug, jumping up on people— endless. Similar habits, such as dog licking that is taken to self-harm levels, that results in the dog developing sores called lick granulomas.

Maybe you’ve seen this: a raised, red sore on a front leg that your dog licks at obsessively. Or perhaps you never seen your dog lick, but the sore is there all the same.

Chances are you told your dog to stop, so they learned to become secretive about licking. Well, these sores are surprisingly common.

Why your dog is constantly licking one spot

Dogs aren’t much different from us. Once the habit starts, it’s hard to break.

The reason is that the dog licking releases feel-good hormones called endorphins. The dog licks, it feels good, and the dog doesn’t want to stop licking.

You may hear your vet refer to the condition as acral lick dermatitis, or ALD, where dogs are licking their legs so much that sores developed.

Studies have shown that when nature’s own morphine-like substances — the endorphins — are involved in ALD, because affected dogs’ self-licking behavior decreased dramatically when the dogs were treated with endorphin antagonists. Since endorphins are released when stress arises, it can be confirmed that licking appears in situations where stress is involved.

Possible Health-Related Causes of the Licking

For most dogs, the habit starts with a trigger in the form of an itch, ache or infection.

It may be that the dog has arthritis or an allergy, and rather than lick the specific spot, the dog chooses a comfortable place to lick — which is often a forearm or paw.

Common triggers:

  • Arthritis

  • Parasitic infection that causes a general itch

  • A bacterial skin infection

  • Ringworm

  • Skin allergies triggering an itch

More than half of dogs with acral lick dermatitis are suspected to have concurrent fear- or anxiety-based conditions or both (e.g., separation anxiety, noise phobia, anxiety-related aggression).

The Derma Dilemma

The problem is that a veterinarian cannot give the owner a specific recipe for a cure. The skin lesions will heal slightly, almost seem like they are going to heal, but then overnight (or during the day while left alone) the lick granuloma is activated by the dog licking the area raw once again.

Even worse, some dogs will simply switch to licking the other leg if you restrict their ability to lick the first leg by wrapping it in a cast, creating an even more annoying problem—now there are 2 lick granulomas!

Does Your Dog Do This? If So, Act Quickly.

If your dog starts to lick obsessively, seek the help of your veterinarian. Getting to the bottom of the cause, and early treatment, is the best way to stop the problem from becoming ingrained.

Alongside treating the lick granuloma, your vet may want to run tests to investigate any underlying problems.

Treatment of Lick Granulomas / Sore Spots in Dogs

Think of this as a 2-pronged attack: tackling both the sores and the underlying cause.

Lick granulomas are frustrating because even with successful treatment, the dog is likely to relapse. To stand any chances of long-term success, therapy must continue for at least 4 weeks after the symptoms have ceased.

Treating the sores:

  • Antibiotics or antifungals

  • Local anesthetic creams

  • Anti-inflammatory medications (to reduce the skin tingle)

  • Bandages or an Elizabethan collar (“cone of shame”)

  • Mood-modifying drugs

Your vet may try one or a combination of the treatments above.

Frustratingly, putting an Elizabethan collar on doesn’t work well because as soon as it is removed, the licking starts again and the dog will activate the lesion all over again.

Addressing the underlying triggers:

  • The first step is to find out what they are. This could mean blood tests, skin biopsies or radiographs.

  • In the case of allergies, starting the dog on a hypoallergenic diet is a great idea, as is testing for environmental allergens.

  • Regular parasite treatments against fleas and mites are crucial to keep parasitic itches at bay.

Ultimately, to find the right solution for your dog, speak with your vet.

Chronic Kidney Disease in Cats

What is chronic kidney disease in cats?

Chronic kidney disease (CKD) is the persistent loss of kidney function over time. Healthy kidneys perform many important functions, most notably filtering the blood and making urine, so problems with kidney function can result in a variety of health problems for a cat. Among the many different kidney diseases that may affect cats, CKD is the most common.

The kidneys are part of the renal system, the body’s system for filtering impurities out of the blood. Urine produced in the kidneys is carried to the bladder by the ureters and from the urinary bladder to the outside world by the urethra.

Clinical Signs and Symptoms of Chronic Kidney Disease in Cats

Cats with CKD may experience a buildup of the waste products and other compounds in the bloodstream that are normally removed or regulated by the kidneys. This accumulation may make them feel ill and appear lethargic, unkempt, and lose weight. They may also lose the ability to concentrate their urine appropriately, and as a result they may urinate greater volumes and drink more water to compensate. The loss of important proteins and vitamins in their urine may contribute to abnormal metabolism and loss of appetite. They may also experience elevated blood pressure (hypertension), which can affect the function of a number of important systems, including the eyes, brain, and heart.

Another cause of lethargy in cats with CKD is the buildup of acids in their blood. The kidneys of cats with CKD may not excrete these compounds appropriately, making affected cats prone to blood acidification, or acidosis, a condition that can significantly affect the function of a variety of organ systems in the body. CKD may also decrease a cat’s ability to produce red blood cells, which can lead to anemia, a reduced concentration of red blood cells in their blood. This may cause their gums to appear pale pink, or in severe cases, whitish in color, and may make them lethargic.


Diagnosis of Chronic Kidney Disease in Cats

To evaluate kidney function, veterinarians will most often turn to blood tests and urine analysis (urinalysis) to evaluate the concentrations of waste products and other components that healthy kidneys normally filter or regulate.

Blood tests can determine the concentration of two important waste products: blood urea nitrogen (BUN) and creatinine, but creatinine is generally recognized as a more specific indicator of kidney function. An increase in the concentration of these compounds in your cat’s blood may suggest that his kidneys are not functioning properly, although these values must be interpreted in light of a number of factors. Dehydration, for example, can cause BUN and creatinine concentrations to increase in spite of the fact that a cat’s kidneys are functioning normally. Ideally, a veterinarian will base his or her interpretation of kidney function on at least two blood samples, obtained within two weeks of one another, from a normally hydrated cat that has fasted for 12-24 hours. The concentrations of other blood components, including various electrolytes (like sodium and potassium), phosphorus, red blood cells, and proteins are also important to evaluate in a cat being examined for CKD.

In a urinalysis, your veterinarian will consider the concentration of the urine, its pH, and the presence of protein, blood cells, bacteria, and other cells that generally should not be found in feline urine, all of which provide important information regarding the health of a cat’s kidneys. It is also important to culture a urine sample to rule out the possibility of bacterial infection of the urinary tract in suspected cases of CKD. Urine samples may be obtained either by collection from a litter box filled with non-absorbent beads designed for this purpose, by catheterization of the urethra (the opening of the urinary tract to the outside world), or by cystocentesis, a technique that extracts a urine sample by passing a very fine needle through the abdominal wall into the bladder. Cystocentesis is generally considered a safe procedure and in most cases will provide the most diagnostically useful sample for analysis.

Other studies that can be useful in evaluating a cat with suspected CKD include imaging studies such as abdominal ultrasound, radiographs (X-rays), and, in some cases, microscopic evaluation of biopsy samples. Given the potential for hypertension in cats with CKD, measurement of a cat’s blood pressure is also an important part of the medical evaluation for this disease.

Treatment of Chronic Kidney Disease in Cats

Although there is no definitive cure for CKD, treatment can improve and prolong the lives of cats with this disease. Therapy is geared toward minimizing the buildup of toxic waste products in the bloodstream, maintaining adequate hydration, addressing disturbances in electrolyte concentration, supporting appropriate nutrition, controlling blood pressure, and slowing the progression of kidney disease.

Dietary modification is an important and proven aspect of CKD treatment. Studies suggest that therapeutic diets that are restricted in protein, phosphorus and sodium content and high in water-soluble vitamins, fiber, and antioxidant concentrations may prolong life and improve quality of life in cats with CKD. However, many cats have difficulty accepting therapeutic diets, so owners must be patient and dedicated to sticking to the plan. It is important to make a gradual transition to a therapeutic diet and to consider food temperature, texture, and flavor. Cats with CKD that go without food for relatively short periods of time may develop significant health problems, so it is crucial to make sure that your cat is eating during a transition to a therapeutic diet.

Controlling hypertension, decreasing urinary protein loss, and addressing anemia are important therapeutic goals in cats that develop these conditions. Hypertension is usually controlled with oral medication, and urinary protein loss may be treated with angiotensin converting enzyme inhibitors. Anemia in a cat with CKD may be treated by replacement therapy with erythropoietin (or with related compounds), which stimulates red blood cell production. Cats with CKD may produce less erythropoietin, and there is some evidence that replacement therapy can increase red blood cell counts. In some cases, blood transfusions, which may be used to restore normal red blood cell concentrations using blood obtained from a donor cat, may be necessary.

Although a number of other therapies, including phosphate binders, potassium supplementation, antioxidant supplementation, alkalinization therapy, and administration of fluids either intravenously or subcutaneously, have the potential to help cats with CKD, these approaches have not been fully validated, and controlled studies are needed to determine whether they offer any benefits. The same is true of hemodialysis (the removal of toxic waste products from the bloodstream by specially designed equipment) and kidney transplantation. These controversial, complex, and expensive treatments offer potential benefits to cats with CKD, but they have not been subjected to studies to prove their effectiveness, so they should be explored with the careful guidance of a veterinary specialist.

Prognosis of Chronic Kidney Disease in Cats

Some cats respond very well to treatment for CKD while others do not, so the prognosis for CKD in affected cats is quite variable. Some studies suggest that cats that lose more protein in their urine have less favorable prognoses. There is evidence suggesting that the earlier CKD is diagnosed and treatment is initiated, the better the outcome with respect to quality of life and survival.

What Is Inappropriate Mentation in Cats? Signs, Causes, and Treatment

Cats, unlike dogs, are discerning and less eager to please their owners. A cat probably won't come when called and rarely will even respond to its name. And while a rare forgotten trip to the litter box or food bowl shouldn't be cause for concern, when a cat seems unusually confused, disoriented, or uncoordinated, a visit to the vet is warranted. It could be suffering from acute sickness, a seizure disorder, or could have been exposed to an environmental toxin, causing a temporary lapse of normal behavior. Any abnormal behavior that continues for more than 24 hours or several times per week could be the result of a serious health issue. Note that a neurologic anomaly can only be diagnosed by a vet.

What Is Inappropriate Mentation?

Inappropriate mentation in cats refers to a host of disorders that can lead to disrupted cognitive functioning. This type of brain misfiring can result in memory loss, incoordination, and basic malfunctioning in everyday cat life. Sometimes these issues are small and acute, caused by temporary injury or inflammation. And sometimes brain disorders can be major, resulting in a compromised quality of life. Assessing the signs and discussing your concerns with your vet are the initial steps in the diagnosis and treatment of mentation disorders in cats.

Signs of Inappropriate Mentation

Cats learn by remembering, but a kitten who suffered an head injury early in life may have damaged the portion of the brain that affects short-term memory. A traumatic brain injury could account for difficulty remembering the location of food bowls or litter boxes. It can also explain an unsteady gait, incoordination, and balance issues. Traumatic injuries can also cause stunted growth. A new pet that seems to be on the smaller side for its age and breed, or a kitten who fails to grow, may have previously succumbed an injury. Seizure disorders can also cause disorientation, pacing, and confusion just following an episode. And old age can result in senility, presenting as forgetfulness, anxiety, and uncoordinated behavior. When witnessed over and over again, this could be confused for a more serious brain injury. Some infections (e.g. feline infectious peritonitis) cause damage to the portion of the brain that controls balance and walking. Affected kittens show severe trouble walking, but can otherwise live a normal and happy life.

Causes of Inappropriate Mentation

There is much debate over whether or not a cat can have a mental illness. If your cat's behavior seems off, it could be ill or could have a more serious problem with its nervous system. "Slowness" can be the result of brain damage due to injury, infections, environmental toxins, seizure disorders, genetic mutations, and even cognitive dysfunction in older cats. In the case of brain injury or infection, a functioning cat is a happy cat. If its setback doesn't interfere with your cat's daily functions, there's little you need to do about it. Get a firm diagnosis so you can take appropriate measures to prevent further seizures and neurological damage.

Cognitive dysfunction (or feline dementia) occurs most frequently in cats greater than 9 years old. If you adopt an older cat and feel like its personality is "off," it could be suffering from senility. In addition to general uncoordinated behavior, cats with dementia can also show unnecessary aggression and may display incontinence.

Diagnosis

At the vet's office, any potential brain injury is treated as an emergency, especially if it was a result of head trauma. So in addition to stabilizing your pet, the vet will perform an exam specifically looking at your cat's neurological system. The vet may determine an MRI or CT scan is needed. In the case of environmental toxin exposure—most specifically lead ingestion (which can cause a slew of neurological issues)— your vet will perform a blood test. If you cat has a seizure disorder, bloodwork is also recommended to rule in/out metabolic causes of seizures. MRI scans are done to rule out inflammatory conditions or tumors. Severe seizure cases require an EEG to measure the electrical activity in your cat's brain. For feline dementia, a vet will discuss your cat's full health history and, if necessary, perform blood tests to rule out other health conditions such liver or kidney disease.

Treatment

Any cat that suffers a potential head injury needs to be examined by a veterinarian. A vet will first stabilize the injured kitty using IV fluids and oxygen. However, a full recovery is not always guaranteed. After the injury, your cat may act different, lose its laser-sharp coordination, or have a change in personality. If this happens, the best you can do is provide a loving and supporting environment, reduce stressors, and eliminate unpredictable events as much as possible. Observe your cat's limitations and provide a lifestyle that works within them. Your veterinarian is an excellent resource for options.

Cats exposed to environmental toxins also need to be inspected by a vet, as the extent of exposure could be fatal. Treatment by a vet may include inducing vomiting, feeding your cat charcoal to absorb the toxin, administering IV fluids and medication, and time to process the toxin. If your cat has a full recovery, any neurological signs should eventually subside.

For seizure disorders (brought on by genetic conditions or other), a vet will prescribe medications to control the seizures. Most cats go on to live happy and healthy lives and any post-seizure temperament disorders should be alleviated.

There is currently no cure for feline dementia. Maintaining a good routine, providing options for exercise, and feeding your cat a high-quality food will help treat further decline.
