1. Print rebate form

To mail in a rebate request, please print out and complete the information in this form. All form fields must be completed to qualify.

2. Enclose an itemized invoice

Enclose an itemized invoice from your veterinary clinic, with invoice number, product purchased, and price and date circled. All doses must be the same product and size, for the same pet and purchased on the same invoice date to qualify. One submission per envelope. If the purchase includes multiple doses for multiple sizes, separate submissions are required by size.
